Procedure Steps

1.Verifies the physician’s order for the prescribed medication.

2. Checks the patient’s medical record for an allergy or contraindication to the prescribed medication.

3. Performs hand hygiene and dons non-sterile gloves. (PPE if exposure to bodily fluids anticipated).

4. Selects the correct medication and compares the label to the order in the patient’s medical record.

5. Checks the expiration date on the medication.

6. Visually inspects the solution for particles, discoloration, or other loss of integrity.

7. Chooses equipment appropriate for the medication and injection site. 

8. Checks or calculates the dose correctly & verifies as necessary.

9. Withdraws the appropriate amount of medication, taps the syringe to expel any air bubbles; removes the needle used for aspiration and disposes of it in a sharps container.

10.Attaches a new needle to the syringe, Loosens but does not remove the cap or protective sheath on the syringe.

11. Verifies the medication at the proper time, in the correct dose, and by the correct route.

12.Confirms the patient’s identity using at least two patient identifiers. (Checks I/D and asks full name).

13. Introduces self and explains the procedure to the patient.

14. Provides privacy by closing the door to the patient’s room and/or drawing the curtain surrounding the patient’s bed.

15. Instructs the patient to sits up and to extends his/her arm and supports it on a flat surface, with the ventral forearm exposed.

16. Examines the patient’s skin and selects an appropriate injection site.

17. Cleans the injection site with an alcohol swab using a circular motion outward, allows the skin to air dry.

18. Stretches the patient’s skin taut, inserts the needle at a 5- to 15-degree angle with bevel up and injects the antigen slowly to form a wheal.

19. Removes the needle at the same angle used for insertion. Avoids massaging or rubbing the site.

20. Circles the test site with a marking pen and instructs the patient not to wash off the circles.

21. Stays with patient for at least 15 minutes to observe any reactions immediately.

22. Discards all equipment appropriately. Places syringe and needle into sharps container without recapping needle.

23. Removes and discards the gloves / PPE.

24. Performs hand hygiene.

25. Assesses the patient’s reaction; (for tuberculin testing, waits for 48 to 72 hours).

26.Documents the procedure, the date, time, name of medicine, dose, route and signature.

27. Reports any unexpected outcomes.
